Key Clinical Points¶
- Normal sinus rhythm (NSR) is defined as a rate of 60-100 beats/min; sinus bradycardia is <60 beats/min.
- Sinus arrhythmia involves P-P interval irregularities >120 ms and is typically asymptomatic and nonpathogenic.
- Inappropriate sinus tachycardia (IST) is characterized by a sinus rate increasing spontaneously at rest or out of proportion to physiologic stress.
- Postural orthostatic tachycardia syndrome (POTS) requires a sinus rate increase of 30 beats/min or >120 beats/min within 10 min of standing without hypotension.
- Distinguishing sinus tachycardia from atrial tachycardia relies on the rate change: gradual increase favors sinus; abrupt onset/offset favors atrial.
- Sinus arrhythmia types include respirophasic (vagal-mediated), ventriculophasic (heart block/PVC related), and nonphasic.
- Ivabradine is a pharmacological treatment for IST.
- POTS management involves multi-modal therapy including high-salt diet, fludrocortisone, midodrine, compression stockings, and exercise.
- Sinus P waves have a specific morphology: positive in II, III, and aVF; negative in aVR; and initially positive biphasic in V.
- Asymptomatic sinus arrhythmia does not warrant further cardiac evaluation.
DEFINITION & OVERVIEW¶
• Normal Sinus Rhythm: ◦ Range: 60-100 beats/min. ◦ Sinus Bradycardia: Defined as rates <60 beats/min (can be normal during sleep or in fit individuals).
• Sinus Arrhythmia: ◦ Definition: Arising from a sinus node origin but with irregularity between P-P intervals of >120 ms. ◦ Note: If different P-wave morphologies are observed, the rhythm is likely due to premature atrial contractions (PACs) rather than sinus arrhythmia. ◦ Clinical Significance: Usually asymptomatic and not considered pathogenic; does not warrant further cardiac evaluation.
• Inappropriate Sinus Tachycardia (IST): ◦ Definition: An uncommon condition in which the sinus rate increases spontaneously at rest or out of proportion to physiologic stress or exertion. ◦ Context: Part of a spectrum of conditions associated with autonomic dysregulation; often affects women in their third or fourth decade of life.
• Postural Orthostatic Tachycardia Syndrome (POTS): ◦ Definition: Symptomatic sinus tachycardia that occurs with postural change from a supine position to standing. ◦ Criteria: Sinus rate increases by 30 beats/min or to >120 beats/min within 10 min of standing in the absence of hypotension.
EPIDEMIOLOGY¶
• Sinus Arrhythmia: Common finding; usually asymptomatic; related to normal physiology. • Inappropriate Sinus Tachycardia: Uncommon condition; often affects women in their third or fourth decade of life. • POTS: Sometimes due to autonomic dysfunction following a viral illness (e.g., "long COVID").
ETIOLOGY & PATHOPHYISIOLOGY¶
Sinus Node Anatomy¶
• Location: Lateral superior aspect of the junction between the right atrium and superior vena cava. • Structure: Located within the superior aspect of the crista terminalis (where the posterior smooth atrial wall derived from the sinus venosus meets the trabeculated anterior portion of the right atrium).
Mechanisms of Sinus Arrhythmia¶
• Respirophasic: Vagal tone is inhibited reflexively during inspiration and restored with expiration. (Also seen with breath-holding in obstructive sleep apnea). • Ventriculophasic: Most often observed with heart block or after a premature ventricular contraction (PVC). The P-P interval is shortened due to a triggered baroreceptor reflex from increased stroke volume/filling time. • Nonphasic: Variations in sinus P-P intervals unrelated to the cardiac or respiratory cycle.
Mechanisms of Inappropriate Sinus Tachycardia¶
• Etiology: Mechanism is elusive; may be related to: ◦ Imbalance between sympathetic and parasympathetic inputs to the sinus node. ◦ Altered membrane automaticity of sinus node cells. ◦ A combination of both.
Mechanisms of POTS¶
• Etiology: Often due to autonomic dysfunction following a viral illness. • Long COVID: Prolonged post-viral symptoms after COVID-19 infection are often associated with autonomic dysfunction and a POTS-like presentation.
CLINICAL FEATURES¶
Symptoms¶
• Sinus Tachycardia: Palpitations, fatigue, dizziness, syncope, chest pain, headaches, and gastrointestinal upset. • Inappropriate Sinus Tachycardia: Fatigue, dizziness, syncope (can be disabling), chest pain, headaches, and gastrointestinal upset.
Electrocardiographic Findings¶
• Sinus P-wave Morphology: ◦ Frontal plane axis: Directed inferiorly and leftward. ◦ Leads II, III, aVF: Positive deflection. ◦ Lead aVR: Negative. ◦ Lead V: Initially positive biphasic morphology. • Differentiation of Tachycardia: ◦ Gradual rate increase favors sinus tachycardia. ◦ Abrupt onset/offset favors atrial tachycardia.
DIFFERENTIAL DIAGNOSIS¶
Sinus Tachycardia vs. Focal Atrial Tachycardia¶
• Gradual Rate Increase: Favors a diagnosis of sinus tachycardia. • Abrupt Onset/Offset: Favors atrial tachycardia (especially from regions near the sinus node). • Clinical Note: Distinction may occasionally require extended ECG monitoring or invasive electrophysiology study.
Inappropriate Sinus Tachycardia vs. Anxiety Disorder¶
• Distinction: Can be difficult to differentiate. • Clinical Clue: Therapy is often ineffective or poorly tolerated in inappropriate sinus tachycardia.
INVESTIGATIONS & DIAGNOSIS¶
- Initial Assessment: Determine if the arrhythmia requires immediate therapy (e.g., for hypotension, impaired consciousness, angina, or heart failure).
- Identify and Treat Reversible Causes: Evaluate based on Table 254-1. • Physiologic: Emotion, exercise, sex, pain, pregnancy. • Pathologic: Anxiety, panic attack, anemia, fever, dehydration, infection, malignancies, hyperthyroidism, hypoglycemia, pheochromocytoma, Cushing's disease, DM with autonomic dysfunction, pulmonary embolism, MI, pericarditis, valve disease, heart failure, shock, alcohol withdrawal. • Drugs/Illicit: Epinephrine, norepinephrine, dopamine, dobutamine, atropine, β-adrenergic agonists (salbutamol), methylxanthines, doxorubicin, daunorubicin, beta blocker withdrawal, caffeine, alcohol; Amphetamines, cocaine, LSD, psilocybin, ecstasy.
- Evaluate for POTS: Recommended for patients with sinus tachycardia. • Criteria: Symptomatic sinus tachycardia with rate increase of 30 beats/min or to >120 beats/min within 10 min of standing without hypotension.
- Identify Sinus Rhythm Characteristics: • Confirm P-wave morphology (Positive in II, III, aVF; Negative in aVR; Biphasic in V). • Rule out atrial tachycardia via rate change analysis.
MANAGEMENT & TREATMENT¶
Physiologic Sinus Tachycardia¶
- Primary Goal: Treatment of the underlying inciting cause.
- Action: Address specific conditions such as anemia, hyperthyroidism, or infection.
Inappropriate Sinus Tachycardia (IST)¶
- Pharmacotherapy: ◦ Beta blockers ◦ Clonidine ◦ SSRIs ◦ Ivabradine
- Catheter Ablation: ◦ Consideration for refractory cases. ◦ Risk: May result in permanent pacemaker requirement due to symptomatic sinus bradycardia or arrest.
Postural Orthostatic Tachycardia Syndrome (POTS)¶
- Non-Pharmacological: ◦ Recumbent exercise and conditioning regimen. ◦ High-salt diet. ◦ Compression stockings.
- Pharmacotherapy: ◦ Fludrocortisone. ◦ Midodrine.
PROGNOSIS & COMPLICATIONS¶
Inappropriate Sinus Tachycardia¶
• Treatment Response: Often ineffective or poorly tolerated. • Ablation Risk: Potential for permanent pacemaker requirement due to bradycardia, arrest, or chronotropic incompetence.
POTS¶
• Resolution: May resolve spontaneously over 3–12 months. • Long COVID: Associated with autonomic dysfunction and POTS-like presentation.
KEY PEARLS & CLINICAL TRAPS¶
• Diagnostic Distinction: Abrupt onset/offset favors atrial tachycardia; gradual increase favors sinus tachycardia. • Clinical Utility: Asymptomatic sinus arrhythmia does not warrant further cardiac evaluation. • Treatment Logic: Treatment for physiologic tachycardia targets the underlying cause (e.g., anemia, thyroid). • Ivabradine: Used in IST when other therapies are insufficient or contraindicated. • POTS Management: Multi-modal approach including salt, fludrocortisone, midodrine, and exercise.
FLOWCHARTS¶
Evaluation and Treatment of Sinus Tachycardia (Figure 254-3)¶
- Start: Patient presents with Sinus tachycardia.
- Step 1: Identify and treat reversible causes (Table 254-1).
- Step 2: Evaluate for POTS. → If POTS identified → Treatment of POTS:
- Recumbent exercise and conditioning regimen
- High-salt diet
- Compression stockings
- Fludrocortisone
- Midodrine
- Step 3: If tachycardia persists and IST is suspected → Proceed to pharmacological management.
- Step 4: Initiate medical therapy with Beta blocker and/or ivabradine.
- End: Consider catheter ablation for refractory cases or specific indications.
